PVIS After-School Club Interest Form

Panther Valley Intermediate School is excited to offer after-school clubs for a second year with transportation! Clubs give students in grades 4-6 the opportunity to try something new, have fun, and spend time with friends while exploring different interests.

This form will help us place students into our upcoming after-school clubs based on their interests and available space. While we will do our best to place students in their preferred club, placement is not guaranteed. If your child is selected to participate in an after-school club, a confirmation letter will be sent home.

We anticipate offering three club rotations throughout the school year, giving students multiple opportunities to participate. Additional details can be found in the form.

Note that Mrs. Palina already sent out interest forms for drama. If you completed an interest form she will be sending a separate bus transportation form home with your child for you to complete. At this time there are no additional slots available for drama.
